Support Plans and EHCPs

What is an SEND support plan?

At Shoreditch Park when we have concerns about a child's progress we will meet with their parents to create an SEN support plan.

Support plans are reviewed at least termly and detail additional interventions and support that we have put in place in order enable a child to make progress. If a child does not make progress despite significant levels of intervention we may decide to make a request for an EHC needs assessment.

What is an EHCP?

An EHC plan is a legal document that describes a child or young person's special educational, health and social care needs. It explains the extra help that will be given to meet those needs and how that help will support the child or young person to achieve what they want to in their life.

EHCP Needs Assessment Process

After an EHC request is received by Hackney Education they have 6 weeks to decide whether to undertake an EHC needs assessment. Please see this flow diagram (in attachments) for more information about the EHCP needs assessment process.

EHCP Pupils

In Shoreditch Park the SENCo meets the parents of EHCP pupils at least once per term to review their provision. An annual review is always held at least once in every academic year. Annual reviews are a great opportunity for all the professionals who work with a young person to get together with the young person and their family to think about what is working well and next steps to ensure that the pupil is able to reach their full potential.

Pupils with an EHCP are always offered additional intervention in line with their specific needs. Please see the intervention section of our website for more information on interventions.

How will my child's progress be shared with me?

Everyone - Shoreditch Park Primary School has three parents' evenings a year. During these meetings all parents /carers have an opportunity to meet with their child's class teacher and discuss progress.

Pupils on SEN register - targets will be shared termly. How this happens depends on the need of the pupil.

Learning needs: Pupils that have learning needs will receive a teacher set target at Parents' Evening.

Speech and Language needs: Parents of pupils on the SALT caseload will receive targets directly from the therapist.

Social and emotional needs: targets will be set through the specific intervention they are accessing.

Children with EHCPs - A SEN support plan meeting once each term to discuss progress towards EHCP outcomes. An annual review will be scheduled once per year.
